In a word...no.

My product is a form processor, it doesn't make forms, it processes them.

What you can do (for example) is make a form to collect the information then
use the autoredirect feature in my script (Pro version) to send the visitor
to a page with a PayPal button on it. In other words, yes you can use my
script to process the form but you need to make the form yourself and you
need to make the page with the PayPal button yourself, then have my script
redirect to it when the form is submitted.
