Parse error: syntax error, unexpected T_ENCAPSED_AND_WHITESPACE  (See more user questions)

Did you fix it? I just noticed your code below. It's been customized quite
a bit. Having a quick look, the email could give a parse error. Change it
to this:

$my_email = "$row_dealerforms[dealer_email]";
