Custom coding request  (See more user questions)

You would need a custom-coded script for your requirements which are too
specific for my product. I'm afraid I can't help you with that.

Presenting a form dependent on the previous form, would need to be custom
coded.

My script does not have a facility to limit submission per IP address.

My script will process anything that is sent to it, so multiple submits
would be processed.

I usually recommend editing the script in a plain text editor, so cannot
offer support for Dreamweaver.

The email from the script is not encrypted.

Sorry I can't be of more help. I would think you might have some difficulty
meeting all your requirements with a stock script.
